Key Responsibilities

• Admissions & Assessments
• Complete Start of Care (SOC), recertifications, and discharge visits
• Perform OASIS assessments accurately and timely
• Care Planning & Coordination
• Develop individualized care plans based on physician orders
• Adjust plans as patient conditions change
• Communicate with physicians, therapists, social workers, and caregivers
• Clinical Oversight
• Monitor patient progress and update EMR documentation
• Ensure medication management and wound care protocols are followed
• Educate patients and families on disease management and safety
• Compliance & Quality
• Maintain regulatory compliance (Medicare, Medicaid, accreditation standards)
• Meet productivity standards (typically 5–6 visits/day)
• Team Collaboration
• Supervise LPNs and aides for delegated tasks
• Participate in case conferences and performance improvement initiatives

Requirements

• Active RN license in Missouri
• Minimum 2 year of Home Health experience (OASIS proficiency required)
• Strong leadership and organizational skills
• Reliable transportation
