Role Description

This is a full‑time, on‑site Clinical Manager role responsible for overseeing Home Health clinical operations while actively supporting the development and preparation of hospice services at Breakthrough Healthcare.

The Clinical Manager is expected to be physically present on‑site each workday to provide hands‑on clinical leadership, oversight, and support. This role is central to ensuring regulatory compliance, maintaining survey readiness, mentoring clinical staff, and strengthening systems that support high‑quality patient outcomes.

The Clinical Manager will lead with clarity and accountability, supporting clinicians while safeguarding compliance and clinical standards. This position is ideal for a seasoned clinician‑leader who understands that growth must be built correctly the first time and who views leadership as stewardship.

Key Responsibilities

  • Provide on‑site clinical leadership for home health operations
  • Support the development and launch readiness of hospice services, including workflows, policies, and staff preparedness
  • Ensure compliance with CMS Conditions of Participation, Missouri state regulations, and accrediting standards
  • Oversee clinical documentation, chart reviews, care coordination, and case conferences
  • Lead and monitor quality improvement initiatives and performance outcomes
  • Mentor, train, and support nursing and interdisciplinary clinical teams
  • Maintain continuous survey and regulatory readiness
  • Address clinical concerns and escalations in real time
  • Collaborate with executive leadership to align clinical operations with organizational growth goals

Qualifications

  • Demonstrated experience in home health clinical management
  • Strong working knowledge of CMS regulations, Conditions of Participation, and survey processes
  • Hospice experience strongly preferred, particularly in program development, compliance, or leadership
  • Proven ability to develop, implement, and sustain clinical and quality systems
  • Experience training, mentoring, and holding clinical teams accountable
  • Excellent leadership, organizational, and problem‑solving skills
  • Bachelor’s or Master’s degree in Nursing, Healthcare Administration, or a related field
  • Active Registered Nurse (RN) license in Missouri or compact state licensure required
  • Knowledge of therapy services is a plus
