Scheduling Team Coordinator – Personal Care Program

Monday–Friday | 7:00 AM – 2:00 PM

Breakthrough Healthcare is seeking a dependable, organized Scheduling Team Coordinator to support our Personal Care Program. This role is critical to ensuring clients receive consistent care and staff schedules are accurate, covered, and communicated clearly.

This is an in‑office position and requires someone who can manage schedules proactively, communicate professionally, and take ownership of daily operations without constant oversight.

Key Responsibilities

  • Create and maintain daily and weekly schedules for personal care staff
  • Ensure authorized hours are fully scheduled and covered
  • Communicate schedule changes promptly to aides and clients
  • Monitor call‑offs and arrange coverage as needed
  • Track attendance, missed shifts, and scheduling discrepancies
  • Work closely with intake, client care, and leadership teams
  • Maintain accurate scheduling records and documentation
  • Follow agency policies and procedures related to scheduling and care delivery
  • Escalate coverage gaps or operational issues appropriately and in a timely manner

Qualifications

  • Prior scheduling experience in home care, healthcare, or a similar setting preferred
  • Strong organizational and time‑management skills
  • Ability to manage multiple schedules and priorities in a fast‑paced environment
  • Clear, professional communication skills
  • Comfortable working independently and being accountable for outcomes
  • Reliable, consistent, and detail‑oriented
