We are expanding our elite clinical therapy team and are seeking skilled Physical Therapist Assistants (PTAs) to join Breakthrough Healthcare. This opportunity is ideal for clinicians who value professional stability, clinical excellence, and work‑life balance.

To support consistency and continuity of care, a minimum commitment of 4 hours per week is required for all staff.

Position Overview

The Physical Therapist Assistant provides skilled physical therapy services under the supervision of a licensed Physical Therapist. This role involves implementing physician‑approved plans of care, monitoring patient progress, and delivering treatment that meets the highest standards of safety, compliance, and quality within the home setting.

Key Responsibilities

• Deliver physical therapy treatments as delegated by the supervising Physical Therapist
• Implement the physician‑signed plan of care accurately and safely
• Observe, document, and report patient responses and any changes in condition promptly
• Collaborate with the PT in assessing functional abilities and supporting care plan revisions
• Complete clinical documentation and progress notes within agency timeframes
• Participate in care conferences to ensure coordinated, patient‑centered care
• Contribute to the agency’s quality improvement initiatives
• Maintain current knowledge of physical therapy practices and trends
• Participate in required in‑service training and professional development

Qualifications

• Active licensure as a Physical Therapist Assistant in the State of Missouri
• Graduate of an APTA‑approved Physical Therapist Assistant program
• Successful completion of the national PTA examination
• Minimum of two (2) years of experience as a PTA preferred
• Ability to work independently in varied home environments
• Strong communication, clinical judgment, and documentation skills
• Reliable transportation and ability to travel to patient homes
