Job Summary
The Intake Specialist is the primary point of entry for all patients and referral sources at Breakthrough Healthcare. You will manage the end-to-end referral process—from the initial phone call to the verified Start of Care (SOC). This role requires a "detective" mindset to ensure insurance is verified correctly, MBIs are captured accurately, and clinicians are scheduled for timely admissions.

Core Responsibilities

Referral Management: Receive and process all incoming referrals from hospitals, physician offices, and family members.

Insurance Verification: Perform rigorous eligibility checks (UHC, Medicare, Medicaid, etc.). Identify "Primary vs. Secondary" payer status and ensure MBIs are correctly entered into the system.

Data Entry & Accuracy: Enter all patient demographics and clinical data into Axxess/MatrixCare with 100% accuracy to ensure clean claims.

Coordination of Care: Act as the liaison between the referral source and the clinical team. Assign the appropriate RN/PT for the Start of Care (SOC) visit.

Physician Orders: Track and follow up on pending physician orders (485s) to ensure they are signed and returned within regulatory timelines.

Communication: Provide high-level customer service to families, explaining our services (Home Health, Hospice, and Daycare) and setting expectations for their care.

Compliance: Ensure all intake activities meet HIPAA and Medicare Conditions of Participation (CoPs).
Qualifications & Requirements

Experience: Minimum of 2 years in Home Health or Hospice Intake.

Software Proficiency: Must be highly proficient in Axxess or MatrixCare.

Knowledge: Strong understanding of medical terminology and the Medicare/Medicaid billing landscape.

Skills: Exceptional phone etiquette, organizational skills, and the ability to multitask in a fast-paced "clean-desk" environment.
