Position Overview
The Licensed Practical Nurse (LPN) provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse. This role is ideal for an LPN who enjoys hands‑on patient care, autonomy, and building meaningful relationships with patients and families. Positions open are Full-time, Part-Time and PRN
Key Responsibilities
- Provide skilled nursing services in accordance with the Plan of Care
- Perform treatments and nursing procedures per physician orders and agency policy
- Observe, assess, and document patient responses to care
- Educate patients and caregivers on disease processes, medications, and home care techniques
- Communicate changes in patient condition to the supervising RN and physician
- Maintain accurate, timely clinical documentation in compliance with Medicare/Medicaid standards
- Adhere to infection control, HIPAA, and safety policies
- Participate in in‑services and ongoing education
Qualifications
- Active Missouri LPN license
- Minimum of 2 years nursing experience (home health preferred)
- Strong clinical and interpersonal skills
- Ability to work independently in the field
- Dependable, organized, and patient‑focused
