Position Overview

The Licensed Practical Nurse (LPN) provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse. This role is ideal for an LPN who enjoys hands‑on patient care, autonomy, and building meaningful relationships with patients and families. Positions open are Full-time, Part-Time and PRN

Key Responsibilities

• Provide skilled nursing services in accordance with the Plan of Care
• Perform treatments and nursing procedures per physician orders and agency policy
• Observe, assess, and document patient responses to care
• Educate patients and caregivers on disease processes, medications, and home care techniques
• Communicate changes in patient condition to the supervising RN and physician
• Maintain accurate, timely clinical documentation in compliance with Medicare/Medicaid standards
• Adhere to infection control, HIPAA, and safety policies
• Participate in in‑services and ongoing education

Qualifications

• Active Missouri LPN license
• Minimum of 2 years nursing experience (home health preferred)
• Strong clinical and interpersonal skills
• Ability to work independently in the field
• Dependable, organized, and patient‑focused
