Quality Assurance (QA) Nurse
Summary
Title:Quality Assurance (QA) Nurse
ID:10522
Department:Clinical Services
Description

Lead with purpose. Work from anywhere. Change care at home.

Some teams check boxes. At Breakthrough Healthcare, we build trust — one visit, one patient, one family at a time. We believe care at home can be exceptional, and it starts with people who care enough to get the details right.

Breakthrough Healthcare is a mission-driven provider of quality care services. Our Medicare-certified, Joint Commission–accredited Home Healthcare division is seeking a remote Quality Assurance Nurse (RN) to join our award-winning team — proud winner of the St. Louis American Best Caregiver Award.

Why this role matters

Great care starts with great people. Our clinicians bring compassionate, reliable care to patients and families in the comfort of home — and they deserve the same support, respect, and encouragement they give to those they serve. In this role, your experience is trusted, your voice matters, and your work shapes the quality of care our patients feel every single day.

You'll partner closely with our Director of Nursing, Nurse Educator, clinicians, intake and operations teams, and leadership — approximately 25 hours per week, fully remote.

What you'll do

  • Review clinical documentation and OASIS assessments so every patient record is accurate, complete, and consistent.
  • Coach clinicians on documentation, care-plan alignment, OASIS, orders, and Medicare home-health standards — with guidance that builds confidence, not fear.
  • Support clinical coordination and visit scheduling so patient care stays timely, organized, and responsive.
  • Collaborate with intake, operations, revenue-cycle, and clinical teams to keep care transitions smooth.
  • Use Axxess Home Health to support quality workflows, OASIS review, and documentation processes.
  • Partner with our Nurse Educator to create staff education, orientation support, and practical clinical resources.
  • Participate in quality improvement, QAPI activities, and survey readiness — strengthening the experience of every patient and clinician we serve.
  • Protect patient privacy and uphold HIPAA, Medicare, and agency standards. These aren't checkboxes here — they're how we honor the trust families place in us.

What you'll bring

Required

  • Active, unrestricted RN license (Missouri license or multistate compact privilege preferred)
  • At least two years of direct home-health field experience as a staff nurse
  • Hands-on experience with OASIS — SOC, ROC, recertification, follow-up, and discharge — plus visit documentation and plan-of-care review
  • Strong knowledge of Medicare home-health documentation, plans of care, orders, and clinical records
  • Experience with home-health coding (ICD-10-CM, PDGM); HCS-D credential a plus
  • Experience with Joint Commission readiness, Medicare Conditions of Participation, QAPI, and clinical audits
  • Excellent written communication, sound professional judgment, and a collaborative coaching style
  • The discipline to work independently, stay organized, and communicate reliably in a remote setting
  • A heart for compassionate care — and a commitment to privacy, teamwork, and continuous improvement

Preferred

  • Experience using Axxess Home Health for clinical records, QA workflows, or OASIS review
  • Experience educating, mentoring, or orienting home-health clinicians
  • Experience supporting clinical scheduling, care coordination, or interdisciplinary operations
